CLAIMS GUIDELINES
Motorcar Insurance Claims
- Report a claim to your servicing officer or to our claims department at 0285533271.
- Prepare the required claim documents and submit to Uniguarantee Insurance Brokerage, Inc.
- Just some things to remember:
- If a third party is involved, you will need to file a police report
- Never admit liability, offer compensation, or have any repairs done without the approval
of the insurance company. - You must report the claim within the allowed number of days from the incident as stated
in your policy or as soon as reasonably able. - Your claim can only be processed upon complete submission of documents.
MOTORCAR CLAIMS LIST OF REQUIREMENTS
Own Damage Claim
- Completed claim form from insurer (UIB will provide this to you)
- Police report or a notarized affidavit of the accident
- Photocopy of the driver´s license
- Photocopy of the Official Receipt and Certificate of Registration (OR/CR)
- Pictures showing the damaged portion(s) and plate number of the insured unit
- Repair estimate
- Stencil of motor and chassis numbers.
Third Party Liability - Property Damage Claim
- Completed claim form from insurer (UIB will provide this to you)
- Police report or a notarized driver’s affidavit of the accident
- Photocopy of the driver´s license for both parties
- Photocopy of the Official Receipt and Certificate of Registration (OR/CR) for both parties
- Pictures showing the damaged portion(s) and plate number of the insured unit and the unit of the insured.
- Repair estimate
- Stencil of motor and chassis numbers.
- Certificate of No Claim issued by the insurer of the third party
Third Party Liability - Bodily Injury Claim
- Completed claim form from insurer (UIB will provide this to you)
- Original copy of the medical certificate in the event of bodily injury or death certificate, or autopsy report in the event of death
- Original copies of the official receipt for medical expenses
- For minor aged victims, a certified true copy of birth certificate to show relationship to the insured.
- Pictures pertinent to the incident
- Police report
- Photocopy of the Official Receipt and Certificate of Registration (OR/CR) for both parties
- Photocopy of the driver´s license
- Affidavit of Release of Claim/Desistance duly signed by the BI claimant
Carnap Claim
- Completed claim form from insurer (UIB will provide this to you)
- Police report or a notarized affidavit of incident
- Photocopy of the driver´s license
- Copy of complaint sheet led with Anti-Carnapping Unit
- Alarm Sheet from PNP anti carnapping unit at camp crame.
- Certificate of Non-Recovery issued by the Anti-Carnapping Unit
- Spare car keys for surrender once claim is settled.
Theft Claim
- Completed claim form from insurer (UIB will provide this to you)
- Police report
- Photocopy of the Certificate of Registration with the official receipt
- Photocopy of the driver´s license with the official receipt
- Photos of the vehicle at the place where theft of accessories occurred. Plate number must be seen.
- Estimate of stolen accessories
- Copy of Anti Carnapping Unit Complaint Sheet
- Alarm Sheet from CHPG and Police Unit from PNP Anti-Carnapping, Camp Crame
- Certificate of Non-Recovery issued by the Anti - Carnapping Unit (can be secured 90 days after the issuance of an Alarm Sheet)
- Vehicle ignition key and spare key upon release of claim check
- Release of Chattel Mortgage, if any.
Fire and Property Insurance
- Report a claim to your account officer or to Uniguarantee Insurance Brokerage, Inc. Claims Department. Your policy will usually require you to inform your insurer within 7 days of the incident.
- Do what you reasonably can to protect the property from further loss and damage.
- Keep the property and evidence of loss untouched until an adjuster can inspect the damaged property. Preserve proof of damage.
- Take photos of the damaged site
- Be prepared to submit the needed documents. UIB will assist you with the documentation.
FIRE AND PROPERTY INSURANCE
General Requirements
- Original or copy of the policy. UIB will assist you with this.
- Incident Report stating the chain of events leading to the loss.
- Photos showing the damaged properties, properly labelled
- Detailed inventory list of damaged items or articles with corresponding quotation / purchase order / official receipt or invoices
- Technical reports from supplier or technical repair provider of the damaged
- Bureau of Fire Certification for loss due to fire
- Other documents that may be requested, depending on the specific type of property insurance and investigation requirements:
- Affidavit of Witnesses
- Bill of quantity
- Lease contract
- Property acquisition documents
- Building, mechanical or electrical layout
- Maintenance log book
- Financial reports
- Inventory reports
- Production reports
- Necessary permits
- Other documents required by adjuster during claim investigation
Marine Cargo

Marine Claims Documents
- Marine Risk Note and endorsement, if any
- Commercial invoice
- Bill of lading
- Packing lists
- Copy of claim against the vessel owner, custom broker, arrastre operator
- Copy of delivery receipts
- Certificate of arrestre for bad order, spillage, leakage, short delivered shipments
- Invoices for repair of damaged items
- Cargo turnover survey
- Other documents as may be required during evaluation of claim
HMO
Call your Account Officer or the EB Claims Unit of Uniguarantee for assistance in arranging for your Letter of Authorization
Guide To Availing From Your HMO Benefits
For Outpatient Services
- Prior to availing of services at any accredited medical service providers, go to the HMO assistance desk to request for a letter of authorization. Present your card and proper identification. If you are using an HMO card issued by your employer, always have your company identification on hand.
- You can call your Account Officer or the EB Claims Unit of Uniguarantee for assistance in arranging for your Letter of Authorization
- Upon proceeding to your scheduled visit to the medical facility, present your LOA and employee ID.
- Medical services availed of that may not be included in your coverage will be charged to you. This must be settled directly with the hospital.
For In Patient Services
- Always present your HMO card as soon as your physician determines that confinement is necessary.
- Prior to your discharge from the hospital, make sure that you fill in your Philheath form and have it signed by your employer. Presenting this form will ensure that you will not be charged for the Philhealth portion of your hospital statement of account.
For Emergency Room Services
- Present you HMO card immediately upon entering the Emergency Room of any accredited medical facility.
- Upon discharge from the ER, the hospital will determine the charges that can be covered by your HMO benefits.
- Portions of your bill that are not covered by your HMO benefits are to be settled directly with the hospital.
- For emergency services availed of at non-accredited facilities, reimbursement may be applied for within the allowable number of days as stated in your policy. However, this will be for further evaluation and approval by your insurer.

Loss of Travel Documents Benefit

- Copy of the Replacement Passport / Travel Tickets

- Original Official Receipts on the expenses incurred for the replacement of Passport / Travel Tickets
Trip Cancellation or Termination
- Proof of advance payment made for travel and accommodation expenses
- Penalties and other irrecoverable pre-paid charges related to the trip
- Legal documents proving trip cancellation with the specified non refundable portion
- Copy of the following documents:
- Copy of Death Certificate, in case of death by insured or direct members of the family
- Full Doctor's report on the emergency of insured or direct members of the family
- Original Physician's declaration of patient unfit to travel
- In case of medical treatment or death of insured's immediate family:
- Proof of relationship with the immediate family member (birth cert,
marriage cert, etc.)
- Original Police report, in case of lost travel documents
